Maintaining
the gains achieved is an essential part of the treatment and recovery
process. That’s why preparing for the return home begins soon
after admission.
Living and cooperating together are the broad goals enabling healthy
re-entry into social life. As the time approaches, the doctor
and social worker confer with the patient and family as well as the staff members
responsible for coordinating the discharge planning.
After
leaving the hospital, it may be advantageous for some patients to attend
supportive group sessions and to continue seeing their therapists.
The hospital maintains a continuing interest in the progress of
former patients and stands ready to assist them during the period
of adjustment following their return to home, family, job, school
and community.
